Obviously this line varies each buddy, but I always got the impression they're keeping an eye on you, not interfering until you go down, like you said. When you meet a buddy in a safe house and ask them to help you, they say something along the lines of "I'll be around". I mean really is he/she following you around staying just out of sight watching the action through binoculars? Supposedly your buddy would drive in soon after, but as you can see they have no proper AI to drive, sit in the passenger seat or even follow you around. Maybe it just attracts nearby patrol trucks.
